Well Identification

A well identification plate is required for certain types of wells under the Water Act (Section 74) and the Ground Water Protection Regulation (Section 8). The well identification FAQ's will help well owners and well drillers determine what types of wells require an identification plate, as well as the attachment and reporting requirements of the plate.

Well Construction/Closure Report Form

The well construction/closure report form may be used to record the construction and/or closure details of a water well. There are currently two forms available for use: well construction/closure report - hand written lithology or the well construction/closure report - check box lithology format. Explanation notes on how to complete the form are available here. For further information contact your regional Ground Water Protection Officer.

Schedule 2

Schedule 2 ( 553kb) contains the reporting requirements for well identification for an existing drinking water supply system (i.e. community) well. Please submit Schedule 2 to:
